Instruction:

The Green Card Entry form should be clearly filled out in the English (Roman) alphabet.

1. FULL NAME
EXAMPLES: Smith, Sara Jane (or) Lopez, Juan Antonio


2. DATE AND PLACE OF BIRTH
Date: Day, Month (in words), Year
   EXAMPLE: 15 November 1961
Place: City/Town, Country
   EXAMPLE: Munich, Germany

NOTE: The name of the country should be that which is currently in use for the place where the applicant was born (Slovenia, rather than Yugoslavia; Kazakhstan rather than Soviet Union, for example).


3. THE APPLICANT’S NATIVE COUNTRY IF DIFFERENT FROM COUNTRY OF BIRTH
If the applicant is claiming nativity in a country other than his/her place of birth, through spouse or parent, this must be clearly indicated on the entry form.
EXAMPLE: Applicant is charging nativity to his wife's country of birth

4. NAME, DATE AND PLACE OF BIRTH of the applicant's spouse and all children.
If you are married, your spouse (husband or wife), even if separated, or even if your spouse does not plan to immigrate, must be included. However, if you are legally divorced, you DO NOT need to list your former spouse.
Any unmarried children under the age of 21 must be listed on the application, regardless of whether they wish to immigrate and regardless of whether they are your natural children, your spouse's children by a previous marriage, or they were adopted, excepting those children who are already U.S. citizens or Legal Permanent Residents.


5. FULL MAILING ADDRESS
This must be clear and complete, because any future mailings will be sent there.
A telephone number and email address are optional, but useful.

6. PHOTOGRAPH
Include a recent "passport style" photographs for each: the applicant,
the spouse and each unmarried child under 21 years with the name and date of birth written on the back.
  • Include the required number of photographs - one for each family member listed on your form
  • Applicant, spouse, or child  must be directly facing the camera.
  • The photo should be taken with the person being photographed
    in front of a neutral, light-colored background.
  • Head covering is not allowed unless required by religion
  • No sunglasses are permitted
  • The head of the person must cover about 50% of the total area of the picture
  • Photo size must be 2 inches x 2 inches (5 cm x 5 cm)
  • Photo may be either color or black and white
  • Photograph must be taken recently (less than 6 months)
